Financial Aid Forms

To complete your financial aid application, we may have requested you submit additional forms.  Below are two lists of forms:

• one for the 2025-2026 academic year (for fall 2025, spring 2026, and summer 2026), and
• one for the 2024-2025 academic year (for fall 2024, spring 2025, and summer 2025).

Please contact us if you are unsure which forms you are required to submit.

• Include your UW-W student ID on all forms and documents.
• Ensure that documents have the required signatures.
• Forms can be faxed, mailed or dropped off in person to the Financial Aid Office. Forms may also be scanned and emailed to the Financial Aid Office at uwwfao@uww.edu.
  • We do not recommend emailing any documents which contain personal information (i.e., social security number, date of birth, driver’s license number, banking information, etc.).
